Bir kedide tarsal ankiloz olgusu ve sağaltımı

Abstract

Kedi ve köpeklerde yüksekten düşme, bir uzvun sıkışması, trafik kazaları gibi travmatik olaylar sonucu, onarılamaz eklem kırıkları, ezilmeler, büyük deformasyon oluşturmuş çıkıklar geri dönüşümü zor ekstremite disfonksiyonuna neden olabilir. Normal hareket yeteneğini kaybetmiş, ya da hareketliliği istenmeyen bir eklemin ankiloz oluşturarak cerrahi operasyonla birleştirilmesi artrodez olarak adlandırılır. Artrodez bir kurtarma prosedürü ve amputasyon için bir alternatiftir. Tarsometatarsal eklemin dorsal ve plantar desteklerini oluşturan yapılar zarar gördüğünde kollateral ligamentler de bundan etkilenebilmektedir. Böyle durumlarda bandaj uygulamaları başarısız olup, daha çok tarsometatarsal eklem artrodezi önerilmektedir. Artrodez sonrası uzvun fonksiyonu normale dönmez ancak çoğu durumda evcil hayvanın, aktif bir yaşam sağlaması için yeterlidir. Sunulan olguda dört buçuk aylık, 2,2 kg canlı ağrılığındaki erkek, melez kedi, sokakta bulunarak, bulan kişiler tarafından, sol arka ekstremitede yapısal bozukluk ve ilgili ekstremiteyi kullanamama şikayetiyle kliniğe başvurdu. Klinik muayene ve radyografik bulguların değerlendirmesi sonucunda açık yara tedavisine ve kedinin genel durumunu normale döndükten sonra operatif olarak sağaltımına karar verildi. Sunulan olguda medialden yaklaşımla plak ile pantarsal artrodez metodu uygulanarak sağaltım sonuçlarının sunulması amaçlanmıştır.

Tarsal ankylosis and its treatment in a cat

Abstract

Traumas such as falling from a height, pinching a limb, traffic accidents in cats and dogs can cause irreversible extremity dysfunction, such as irreparable joint fractures, crushes, dislocations with large deformations. Arthrodesis is the surgical immobilization of a joint that has lost its ability to move normally or that needs to be immobilized by fusion of bones. Arthrodesis is a salvage procedure and an alternative to amputation. When the dorsal and plantar structures forming the tarsometatarsal joint are damaged, the collateral ligaments may also be affected. In such cases, bandage applications may fail, tarsometatarsal joint arthrodesis is recommended. After arthrodesis, the function of the limb does not return to normal, but in most cases it is sufficient for the patient to maintain an active life. In the presented case, a four-and-a-half-month-old male, crossbred cat weighing 2.2 kg was found on the street and brought to the clinic with the complaints of structural defect in the left hind extremity and inability to use the extremity, by the people who found it. After clinical and radiographic examination, it was decided to treat the open wound, and when the general condition was normal, surgical treatment was decided. In this case, it is aimed to present the treatment results by applying the pantarsal arthrodesis method with a medial approach.
